Waiver

  I certify that I am the parent or legal guardian for the minors listed on this form and I consent to their participation in the activities of AYM West (Adventist Youth On A Mission West).

  I hereby accept and assume all risks, known and unknown, and assume all responsibility for any losses, costs and/or damages incurred by these listed. 

  I hereby release from liability the Central Valley SDA Church, and AYM West, its board, attendees, and affiliates, for any and all claims, demands, losses, or damages on account of any injury. I understand and accept the normal risks in life, including but not limited to, coronavirus and scorpions.

  I consent to the use of my and my child’s image in promotional material by AYM West and it’s affiliates.
